A small group of activists known as the Ghost of Icarus is attacking the mega-corporation Daedalus Group. While tending to some of the bombing victims, Black Jack meets Dr. Kiriko, better known as Death Incarnate, who, like him, demands large sums of money from his patients but instead of saving them, provides them a painless death. When the activist group asks both physicians to work together, Black Jack finds himself trapped in a web of life, death, and betrayal while striving to preserve his patients' lives and prevent a biological war that might wipe humanity out.
